Terms and Conditions for Online Nutrition Counseling

Effective Date: 1/20/26
Practice/Provider: Off The Wall Food And Nutrition
Registered Dietitian Nutritionist: Alicia E Woll, MS, RD, LD, LMNT

1. Agreement to Services

By scheduling or participating in nutrition counseling services, you acknowledge that you have read, understood, and agreed to these Terms and Conditions and any applicable consent and privacy documents provided by the Practice.

2. Nature and Scope of Services

Nutrition counseling may include nutrition assessment, education, medical nutrition therapy when appropriate, goal setting, meal-planning guidance, behavior-change strategies, and monitoring of nutrition-related concerns.

Nutrition counseling does not replace medical evaluation, diagnosis, emergency medical care, or treatment by your physician or other healthcare professionals.

3. Telehealth Services

Services are provided remotely through video, telephone, secure messaging, electronic portals, or other approved communication technologies, as appropriate.

Telehealth has limitations. These may include technology failures, interrupted connections, limitations in performing physical examinations or obtaining measurements, and privacy risks inherent in electronic communications.

If technology prevents an appointment from being completed, the Practice may attempt another communication method or reschedule the appointment.

4. Client Location and Provider Authorization

You must provide your current physical location at the time of each telehealth appointment when requested.

Services will be provided only when the Provider is legally authorized to provide nutrition services to a client located in that jurisdiction.

If you are located in a jurisdiction where the Provider is not authorized to practice, the appointment may need to be cancelled, rescheduled, or transitioned to an appropriately authorized provider.

Traveling or temporarily relocating may therefore affect whether your scheduled appointment can occur.

5. Client Responsibilities

You agree to provide accurate and complete information regarding your health history, medications, supplements, allergies, laboratory results, diagnoses, dietary practices, and other information relevant to nutrition care.

You are responsible for informing the Provider of significant changes in your health or treatment.

You should continue appropriate care with your physician and other healthcare professionals and should not discontinue or modify prescribed medications or medical treatment solely on the basis of nutrition counseling unless instructed by the prescribing clinician.

6. Results Are Not Guaranteed

Nutrition outcomes vary between individuals. Recommendations may depend on medical conditions, adherence, medications, genetics, lifestyle, and other factors.

The Practice does not guarantee weight loss, improvement of laboratory values, resolution of symptoms, prevention or cure of disease, or any other specific outcome.

7. Emergencies

Online nutrition counseling is not an emergency service and is not intended to diagnose or manage medical emergencies.

If you believe you are experiencing a medical emergency, call 911 or seek immediate evaluation from an appropriate emergency medical service.

Electronic messages, portal messages, emails, and appointment requests should not be used for emergencies.

8. Privacy and Electronic Communications

The Practice will take reasonable measures required by applicable law to protect the privacy and security of your health information.

Electronic communication and telehealth technologies may nevertheless involve privacy or security risks. You are responsible for participating from a location where you feel comfortable discussing confidential health information and for protecting access to your own devices, passwords, email, and accounts.

The Practice's separate Notice of Privacy Practices and/or Privacy Policy, when applicable, provides additional information regarding the handling of health information.

9. Fees, Insurance, and Payment

Fees: Depending on the length and type of service, fees range from $ 25/month to $250/session.

Insurance: We accept insurance from a variety of insurers that are subject to change. You are responsible for any copays, deductibles, and additional fees that the insurance company may charge based on your individual coverage.

You are responsible for understanding your insurance benefits and for amounts not covered by your health plan, subject to applicable law and contractual obligations.

Payment is due at the time of service.

10. Cancellation and No-Show Policy

Appointments must be canceled or rescheduled at least 24 hours before the scheduled appointment.

Late cancellations or missed appointments may result in a $75 fee, except where prohibited by applicable law or contract.

11. Communication Between Appointments

For non-urgent questions, you may contact Practice via secure, HIPAA-compliant messaging and email. To be provided during initial appointment.

Typical response time: within 24 hours during business hours.

Electronic communications are not continuously monitored and should not be used for urgent or emergency concerns. If you are experiencing an emergency, immediately seek local care or contact your local emergency services at 911.

12. Recording of Sessions

Neither the client nor the Provider may record an appointment without appropriate prior consent and compliance with applicable law. AI software may be used during the appointment solely for transcribing and summarizing the appointment.

13. Minors and Legal Representatives

For clients who are minors or who require a legally authorized representative, appropriate consent or authorization must be obtained in accordance with applicable law before services are provided.

14. Termination of Services

You may discontinue nutrition counseling at any time.

The Practice may discontinue services when appropriate, including when services are outside the Provider's scope of practice, telehealth cannot be provided safely or legally, required payments remain unresolved subject to applicable law, or another level or type of care is needed.

When clinically appropriate, the Practice may recommend follow-up with another healthcare professional.

15. Changes to These Terms

The Practice may revise these Terms and Conditions as laws, professional requirements, technology, or Practice policies change. The version in effect when services are provided will apply, subject to applicable law.

16. Acknowledgment and Consent

By signing electronically or otherwise affirmatively accepting these Terms, you acknowledge that:

  • You have read and understood these Terms and Conditions.

  • You understand the benefits and limitations of receiving nutrition counseling through telehealth.

  • You understand that your physical location may determine whether the Provider can legally provide services to you.

  • You have had an opportunity to ask questions.

  • You voluntarily consent to receiving nutrition counseling through telehealth.
